We are enrolling new patients and look forward to caring for you and your family.

All patients wishing to enrol must complete an enrolment form. In order for the enrolment to be successful, the NZ Ministry of Health requires all patients to provide proof of identity (regardless of NZ Citizenship status) e.g., Passport, NZ driver’s license and eligibility if applicable (i.e., permanent/resident/work visa). If you currently hold a work visa it must be a minimum of 2 years in order to meet eligibility requirements.

For patients under 16 years of age, a parent or legal guardian must complete the form.

Please note: all online enrolments can take up to 5 business days to be processed and completed. If you have requested the transfer of your notes from your previous medical centre, it may take up to an additional 5 business days to be received at our clinic.

Print an enrolment form

If you prefer to complete a physical form, please download our enrolment form or casual enrolment form and print it from the comfort of your home.

Once completed, please either email it to [email protected] or drop it off to one of our friendly receptionists along with your supporting documents. Please note, all shaded areas are mandatory and a signature is required.

Therefore, you will need to print and sign the form.
