In addition, a range of fully funded services is also available through our Pharmacist.
Our Health and Wellbeing Team can support you with:
- Healthy eating and physical activity guidance
- Medication reviews and management
- Stress management strategies
- Goal setting and personalised action plans
- Chronic pain management
- Support with medically unexplained symptoms
- Help navigate grief, sleep difficulties, low mood, anxiety, and stress
- Managing long-term health conditions
- Assistance with family violence or relationship concerns
- Support for children with behavioural challenges

Tessa Hafoka – Health Improvement Practitioner
Tessa is a qualified Occupational Therapist and is passionate about helping people reconnect with what matters most to them—whatever that looks like in their own lives.
She has previously worked as a Health Improvement Practitioner (HIP) in another clinic and brings a wide range of experience in mental health and social services, as well as lived experience of disability and chronic pain.
As the HIP, Tessa offers brief, practical sessions (around 30 minutes) focused on developing useful strategies and linking people with external services that support wellbeing.
She can support people with a wide range of concerns, including:
- Stress, feeling overwhelmed or anxiety
- Sleep difficulties
- Anxiety, depression, or low mood
- Grief
- Adjusting to significant life changes and navigating unexpected events
- Living well with physical health conditions or a disability
- Relationship challenges
- Alcohol or drug use
- Adjusting to change
- Parenting
- Prenatal and Postnatal wellbeing
- Family violence
- Chronic pain
- Maintaining healthy habit changes
- Maintaining your health and wellbeing while supporting or caring for someone else
The HIP service is FREE for all registered patients of Henderson Medical Centre, including children and young people. To make an appointment, you can call reception or speak with your doctor or nurse. Doctors and nurses may also refer you if they feel the service would be helpful.
Appointments are available in person or by phone, and can be pre‑booked or arranged on the day.
Simone Llewellyn – Health Coach
Just like sports coaches, the role of a health coach is to inspire confidence in you, share knowledge with you and find ways to motivate you to achieve your goals. A Health Coach works alongside your GP, nurse, and wider healthcare team to support your overall wellbeing
She can work with you on a range of concerns, including:
- Managing and understanding conditions like diabetes, high cholesterol and gout
- Eating and exercise habits
- Becoming smokefree or reducing smoking
- Understanding your medications
- Build confidence and self-motivation
- Support with setting goals and plans, towards improving your health
- Get linked with local support groups, community support workers, exercise classes, food banks, social services, or mental health resources.
Michele Walters – Pharmacist
Providing expert medication support for safer, more effective care.
Michele Walters is our Pharmacist. With many years of clinical experience, Michele brings a wealth of knowledge to support both patients and the wider healthcare team in General Practice.
She works closely with patients and whānau to provide clear, accessible advice around medications, helping ensure they are used safely and effectively as part of ongoing health management.
You may be referred to Michele by your GP or nurse if:
- Your medications are complex
- You are experiencing side effects
- You have recently been discharged from the hospital
- You’ve been diagnosed with a new medical condition and require medication support
Michele can help with:
- Explaining medications in simple, easy-to-understand language
- Monitoring and reviewing your medications
- Making clinically appropriate changes to your medication when needed
- Ensuring everyone involved in your care is aware of your current medication plan
Lucy Adair – Care Coordinator
Supporting patients with complex health needs through connection and collaboration.
Lucy Adair is the Community Care Coordinator at Henderson Medical Centre and a valued member of the Comprehensive Primary Care Team. A proud West Aucklander and registered nurse.
Lucy brings extensive experience across a wide range of specialties, including neurosurgery, neurology, maternal mental health, infant and child health, and maternity care in both community and hospital settings.
Lucy works in partnership with patients, GPs, nurses, and other providers to support improved health outcomes, particularly where care needs are complex or involve multiple services.
Your GP or nurse may refer you to Lucy if:
- You require referrals to external health providers
- Your care involves coordination between multiple specialties
- You need help navigating or communicating with health and social services
- You would benefit from developing a personalised wellbeing plan to support your health and resilience
